Form-Based Field Reporting Software

FormStream is a highly flexible, interoperable and customizable software solution that significantly simplifies the report writing and approval process.  FormStream can deliver significant benefits and efficiencies in both a mobile and non-mobile environment.  Using FormStream, agencies can create and manage customized reports, complete reports more easily with auto-population and user tools, process reports and import and export data all from vehicles using computers with wireless network connectivity.  Where network connectivity is not available, data is saved and automatically sent when connectivity is available.  For agencies choosing FormStream without mobile connectivity, reports can be transferred at the end of the day using a flash drive.

Citizen Generated Reports

FormStream’s eGovernment Interface allows citizens to complete Incident Reports via on online portal.  Cities and counties can provide a link within their site launching a fill-able report citizens can complete.  Once the report is submitted, it is routed to the appropriate personnel and if approved, the report is sent to the appropriate module in the RMS. 

Interacting with that citizen, personnel can request additional information to complete processing and once the report is processed, a notification can be sent either to the citizen’s email or to assigned officer.  FormStream can accommodate a variety of processes, which can be set up by SpireLine’s Professional Services or by the agency themselves.

AMBER Alerts

FormStream help agencies deploy AMBER Alerts more quickly.  Using FormStream, agencies can enter collected data into the forms database to facilitate the activation process.  The reports can then be sent and routed more quickly and to more organizations and agencies in less time than conventional paper methods.  When an Alert is needed, every minute counts and FormStream is there to streamline the collection, routing and delivery of critical information.
